Actions Upon the First Assessment

Upon the first assessment, the nurse is required to act in a manner that will put the patient in the best condition for nature to act upon him. This is based on the assumption that environment is essential for the health of the patient. The nurse in this case is required to focus on the manipulation of the environment. This implies that the nurse will seek to improve the environment of houses to achieve prevention of illness, promotion of health, observation of sick, attention of nutrition, and attention to patients and their needs (Awalkhan, 2016). 

The main things that the nurse would seek to achieve to assist this patient is improvement of the lighting and ventilation of the house. This may be achieved by opening the windows and drawing the curtains. It will also be important to consider how the broken air conditioner. Another important action that will need to be taken is on providing adequate supply of nutritious food. The nurse also needs to ensure that patient’s clothes are cleaned.

The Environmental theory by Nightingale is based on some assumptions that health and sickness are influenced by natural law, nursing is a unique field unlike medicine which is both a science and art. Nightingale indicated that the disease process should not be the main concern for the nurses but rather the nurse should be focused on the individual in the environment. The next main issue as proposed by Nightingale should be the availability of clean water; water is regarded as sanitary. The nurse should ensure that there is a separation of drinking water from sewers. Any drinking water should be purified before consumption to kill disease causing microorganisms. The next concept required is on drainage where the pipes and sewers need to be effectively drained.  The fourth concept is on cleanliness where the nurse is expected to ensure that there are open windows, proper dusting, appropriate waste disposal, clean linens, clothes, carpets, and floor. The other main concept as proposed by Nightingale is on the lighting of house. Houses with a lot of darkness are regarded are unhealthy and poorly circulated. The theory proposes that the sun and light are the important for the appropriate healing of the patients (Smith, & Parker, 2015).

Physical Environment 

The physical environment where Mrs. Ada lives is characterized by limited airflow and light, and no conditioning. It is recommended that the nurse advise on how to improve these conditions as a means of restoring health of the patient and prevent disease or injury. Among the issues to adhered to is aeration. The house of the patient needs to be maintained as pure through ambient air and heating. For Mrs. Ada, the windows should be opened and curtains drawn during daytime. The broken air conditioner should be fixed to continue supporting the provision of clean air and maintaining the appropriate temperatures.


From the physical assessment of the conditions at home, it was noted that there was no food. Food is essential requirement to assist the patient maximize their full potential as recommended by the Nightingale’s theory. The patient requires to have plenty supply of food. The food should be adhering to dietary requirements for diabetic patients and high blood pressure. The type of food taken is very crucial for improvement of health for the patient.

Personal cleanliness is important element of the environment that affects the health status of the patient. It is important that the patient maintain personal hygiene. This will reduce the risk of infections. The patient should get an extra set of clothes and ensures that they clothes are properly cleaned. Maintain hygiene will involve the appropriate management of the wound. The wound will be washed, disinfected, and bandaged to prevent any infection that might occur from poorly managed wound and also facilitate fast healing.
